This is a plain-language summary of how Chaperone Health handles protected health information (PHI) where HIPAA applies. Our full Notice of Privacy Practices is being finalized and will govern in case of any difference from this summary.
1. Our commitment
Where Chaperone operates as or on behalf of a covered entity, we are committed to safeguarding PHI and to using and disclosing it only as permitted by law and by our agreements with the organizations we serve.
2. How PHI may be used and disclosed
PHI may be used to coordinate peer support and care navigation, to operate and improve the service, and for purposes permitted or required by law. Intelligent Guidance (our AI) assists with operational tasks under safeguards; it does not replace the human relationship at the center of care.
3. Your rights
Individuals generally have rights to access their PHI, request corrections, ask for restrictions, and receive an accounting of certain disclosures. The final notice will describe how to exercise each of these rights.
4. How we protect PHI
We apply administrative, technical, and physical safeguards designed to keep PHI confidential and secure. You can read more about our posture on the Trust center and on our dedicated HIPAA page.
5. Changes to this notice
We may update this notice as our practices or the law evolve. The final published version will include an effective date and instructions for obtaining a copy.
